  • Ensures family monitoring to improve screening and early care;
  • Routes requests for services to facilitate access to care and services for children and families;
  • Ensures continuity of services;
  • Acts as a relay between the people working with the family to ensure that communication is optimal;
  • Promotes partnership between health services and family doctors as well as educational communities;
  • Works with a team of professionals from several sectors of youth and adult mental health.

The nurse does not offer therapeutic treatment.

How to get the services?

  • Families can ask the worker for a reference in their file or that of their child.
  • Requests can also come from the family doctor.

Families can also contact the HoPE Program team directly to verify if they are eligible for the program.
