In , HIV and STI screenings are still more frequent and diagnosed than in mainland . This observation is, however, accompanied by an upward trend in AIDS discoveries, a sign that certain diagnoses are still made late, despite the overall improvement in access to screening. Concerning HIV, screening is 123.6/1,000 people. The screening rate is higher among women than men, except among those aged 50 and over. – HIV infections and AIDS –

In 2023, in Reunion, the HIV screening rate (people screened at least once a year per 1,000 inhabitants) will increase sharply, going from 91.0 in 2022 to 123.6. This increase is also observed in mainland France (excluding IdF) but to a lesser extent. The screening rate is also lower there (just above 75/1,000 inhabitants)

Screening rates are higher among women, in all age groups except those over 50. This rate is highest among women aged 25-49 (264.6/1,000 inhabitants).

The rate of HIV serology carried out in Reunion is 171/1,000 inhabitants, a sharp increase compared to 2022. It is higher than that of mainland France but lower than those of other overseas regions (except Mayotte). The positivity is stable (0.6 per 1,000 serologies performed).

In Reunion, the number of HIVTests carried out increased throughout 2023 compared to 2022 and in all age groups. The strong increase in November/December is also found in certain regions of France. It is the 25-49 age group who benefited the most from the system.

Other screening data are available thanks to a diversified offer. These include rapid diagnostic orientation tests (TROD) carried out by associations in community settings. In 2023, for the whole of France, more than 51,000 HIV TRODs have been carried out, including 0.75% which turned out to be positive (source: DGS, ARS).

Furthermore, around 53,000 HIV self-tests were sold in 2023 by pharmacies (entire France), including online sales. Finally, around 17,000 self-tests were distributed by community associations, a decrease compared to 2022 and 2021 (28,735 and 32,531 respectively).

– Discoveries of HIV seropositivity –

The number of discoveries of HIV seropositivity, corrected for under-reporting, missing data and reporting delays in Reunion was 57 in 2023. The number of discoveries of seropositivity decreased between 2021 and 2022 and then increased again in 2023.

In 2023, in Reunion Island, findings of HIV status mainly concerned heterosexual people born in France.

In Reunion, compared to 2022, we observe an increasing trend in discoveries of seropositivity at an early stage and at an advanced stage and a decrease in discoveries at the late stage. The majority of HIV findings concerned cis men (76%), born in France and engaged in heterosexual practices.

The number of people living with HIV in Reunion without knowing their HIV status was estimated at 119 (68-170) at the end of 2023.

In Reunion, the median time (25% and 75% quantiles) between contamination and diagnosis was 2.5 years (0.8-5.9) for all people diagnosed in 2023, regardless of the place of contamination. . Among migrants unaware of their HIV status upon arrival in France, the median time (25% and 75% quantiles) between arrival and diagnosis was 0.5 years (0.1-1.5).

– AIDS diagnostics –

The number of AIDS diagnoses in Reunion, corrected for under-reporting and reporting delays, was estimated at 9 (95% CI: [4-14]) in 2023. In Reunion, the number of AIDS diagnoses has been increasing since 2021, contrary to national trends.

The vast majority of people diagnosed at the AIDS stage did not know their HIV status beforehand (81.8%) and none had received antiretroviral treatment in the 3 months preceding the diagnosis of AIDS. These diagnoses mainly concerned people engaged in heterosexual relationships.

– Sexually transmitted infections (STIs) –

Since 2020, in Reunion, the screening rate for Chlamydia trachomatis infections has been increasing (80.3/1,000 inhabitants in 2023 compared to 68.7/1,000 inhabitants in 2022). This is particularly the case in women, in whom they are detected much more often than in men (except in those aged 50 and over).

Overall, the diagnosis rate of Chlamydia trachomatis infections remains stable over time. The diagnosis of Ct infection is, however, increasing among men in the 15-25 and 26-49 age groups.

Gonococcal infections

The screening rate for gonococcal infections has been relatively stable since 2019 in Reunion. Among women (aged 15 to 25 and 25 to 49), it is almost 3 times higher than the regional average.

The diagnosis rate of gonococcal infections is increasing slightly in Reunion. It is 4 times higher than the regional average for women aged 15 to 25 and 2 times higher than the average for women and men aged 24 to 49.

– Syphilis –

The syphilis screening rate has been relatively high in Reunion since 2019. It is among women aged 15 to 25 and 25 to 49 that these rates are the highest, almost 3 times higher than the regional average. .

After a decline between 2019 and 2022, the syphilis diagnosis rate increases slightly again in 2023. It doubled among women aged 15 to 25, going from 14.4 per 1,000 inhabitants in 2022 to 32.1/ 1,000 inhabitants in 2023, almost times the regional average (12.3/1,000 inhabitants).

– Condom sales data –

In Reunion, 911,714 male condoms were sold in supermarkets and pharmacies (excluding drugstores) in 2023 (source: Public Health France). This figure is stable compared to 2022.

In addition, condoms were made available free of charge by Public Health France, the regional health agency (ARS) La Réunion, CoreVIH and the General Council.
