EFAS is the German-speaking acronym used to talk about the uniform financing of ambulatory and inpatient services, on which the Swiss will have to express themselves on November 24. Technically, this reform of the LAMal plans to modify the way of financing the care of patients in hospital. Currently, all outpatient care (without hospitalization) is covered by health insurance premiums. Hospitalizations are the responsibility of the cantons (55%) and premium payers (45%).

A distribution key that poses a problem for the majority of players in the health system. To better balance the burden between cantons (taxes) and insurers (premiums), Ruth Humbel, former national councilor (PDC/AG), tabled an initiative in December 2009 to review this financing. The goal: to relieve premium payers. After an interminable legislative process of fourteen years and numerous modifications, parliament approved the project on December 22, 2023. In the process, long-term care (EMS and Spitex) were integrated into the reform, under pressure from the cantons. The latter currently cover 46% of this care, compared to 54% for insurers.
