Heart failure: an online tool to coordinate care in Belgium

The Belgian Working Group on Heart Failure (BWGHF) celebrated its 20th anniversary. On this occasion, the multidisciplinary care pathway for heart failure was officially presented last week. This tool aims to support all doctors and healthcare professionals in the monitoring and treatment of patients suffering from heart failure.

Cardiovascular diseases remain the main cause of mortality in Belgium, and heart failure plays a significant role. Monitoring and treating patients with heart failure has been a challenge for doctors for decades.

It is in this context that this multidisciplinary course was designed. The site dedicated to this course is presented as an online platform offering practical advice for the diagnosis, treatment and multidisciplinary monitoring of patients with heart failure.

The platform is aimed at all stakeholders involved in patient care: patients themselves, their families, general practitioners, specialists, nurses, pharmacists and physiotherapists. The objective is to improve collaboration between all healthcare providers in order to optimize the quality of care and the prognosis of patients in Belgium.

The information and recommendations offered are based on the most recent guidelines of the European Society of Cardiology (ESC), as well as scientific knowledge and practical clinical experience. Through this, the working group hopes that patients with heart failure will benefit from optimal and uniform monitoring, advice and treatment, regardless of the professionals involved.
