Job Description
Within the Pôle Solidarités, which leads social action in the Hauts-de-Seine region, the Territorial Solidarity Department is developing a range of solidarity services from early to old age. In order to guarantee users the proximity of its services, the Solidarity Department relies on thirteen territorial solidarity services spread across the territory. The thirteen entities bring together different medical and social services with professionals intervening in a transversal approach (social action and integration, maternal and child protection, family planning, child protection). True markers of the Department's innovation in terms of local solidarity, they are designed to be places open and accessible to all audiences, which ensure:
– Support for users in their efforts to access rights and support digital procedures.
– Multidimensional assessment of all vulnerabilities and social and medico-social needs of the public.
– The construction with users of support pathways, taking into account the person and the family in all dimensions of their life.
The organization of each territorial solidarity service reflects this path logic with a structure around three units, led by unit managers:
– Reception Unit
– Evaluation Unit
– Support Unit
The action of the Department of Territorial Solidarity is part of an interdepartmental plan for social and medico-social organization in Yvelines and Hauts-de-Seine in favor of early childhood, families, the elderly, and disabled people. and the most fragile.
Within the Evaluation unit of a Territorial Solidarity Service, under the supervision of the head of the unit, you contribute to the evaluation of situations of danger and/or risk of danger, vulnerability, among children, adults and families. You participate, within the framework of an ethical and deontological approach, to create the conditions so that people, children, families and groups have the means to be actors in their development. You will be able to rely on the technical skills of your unit's Child Protection Referral Officer.
Your activities:
1. Conduct or contribute to conducting evaluations relating to:
‐ Situations transmitted by the Concerning Information Collection Unit. Define the evaluative strategy and implement the assessment of the minor's danger and/or risk of danger based on concerning information. Make recommendations in terms of child protection.
‐ Situations to be assessed urgently and identified within the framework of the unique permanence of solidarity: assess and recommend responses adapted to people in situations of rupture, emergency and social fragility, particularly in terms of prevention rental evictions, for victims of domestic violence, situations of food or accommodation breakdown. Implement temporary placement orders (OPP) for minors in danger.
‐ Any other adult situation at risk of vulnerability within the framework of programmable assessments (elderly people for example).
2. Involve families in the analysis of their problems and solutions.
Write professional writings related to this activity, in particular reports transmitted to the judicial authority.
Represent your service in court.
3. Ensure the transfer of situations that require support to other services in the department or partners.
Mobilize the network of partners and participate in technical bodies around individual situations.
Network with all stakeholders, institutions, professionals and associations.
4. Participate in meetings and multidisciplinary bodies in your department.
Contribute to departmental projects.
