Fruit of close collaboration between the urology and physical medicine and rehabilitation services (MPR), a multidisciplinary consultation in neurology now exists at the CHU, meeting a clearly identified need. Dr. Maxime Vallée, urologist and coordinator of this initiative, underlines its major interest.

Neurology is a specialized branch of urology which is devoted to vesico-sphincter and pelvic disorders resulting from dysfunctions of the nervous system. For example, it concerns medulla -injured patients (paraplegic or tetraplia) following an , as well as those with neurological diseases such as multiple sclerosis and many more. These patients may have difficulty urinating and to control the bladder. “” The bladder operates in a more or less reflex manner depending on the type of neurological pathology, a phenomenon designated by the term “neurogenic bladder”. This problem requires rigorous monitoring, both by urologists and by physician of physical medicine and rehabilitation “Explains Dr. Maxime Vallée. Previously, these patients were seen separately, at times and in distinct places, by urologists and physician of physical medicine and rehabilitation, without particular coordination.

In order to optimize the care of patients in neurology, Dr. Maxime Vallée, specialists in physical medicine and rehabilitation, Valentine Gilquin and Laura Mainini, and Doctor Amélie Dos Santos, neurologist, initially set up, in 2023, a multidisciplinary consultation meeting to discuss the files of complex patients. “” These meetings constituted a step in the formalization of the care of patients in neuro-urology, which is classically a matter of appeal activity ». To go further in improving this care, Dr. Maxime Vallée then proposed the creation of a multidisciplinary consultation. Strongly involved in neuro-urological issues, he has long aspired to offer a coherent care path. The Poitiers University Hospital had not previously had a dedicated sector allowing Urologists and MPR doctors to question and examine the patient simultaneously. This consultation allows specialists in the two disciplines to jointly assess the patient, to deal with neurological problems by adjusting the entire management according to the underlying pathology, in order to bring him the solution. “” The primordial interest of this consultation lies in the simplification of the care path for the patient and the improvement of our respective care. It offers us the possibility of making concerted decisions, in the presence of the patient, thus responding directly to his questions and his fears. This also allows us to discuss our respective knowledge and offer a better quality of care while taking into account all the parameters specific to these pathologies which reach the nervous system “Specifies Dr. Maxime Vallée.

The multidisciplinary consultation of Neuro-Urology, launched in October 2024, is held every three months within the Service of Physical Medicine and Rehabilitation, located in the Maurice Salles pavilion, on the Milertrie site, where the premises are more suitable. This also makes it possible to benefit from the support of Mrs. Stéphanie Posson, a specializing in MPR.

« Until 2022, the patients of and Poitou-Charentes suffering from neuro-urological disorders were rather oriented towards the CHUs of Bordeaux or Nantes. We are delighted to now be able to offer a coherent and structured remedy career nearby “Concludes Dr. Maxime Vallée.
