Quality of care: psychiatry must improve

The High Authority for Health publishes the results of the quality and safety of care indicators collected in 2024 and takes stock of the certification of establishments. Certain weak points are highlighted in psychiatry. Developments are expected via the Mental Health and Psychiatry 2025-2030 program.

The collection of health care quality and safety indicators and the certification of health establishments are two essential systems put in place by the High Authority for Health to improve the quality of care. At the end of the year, the agency presents on the Qualiscope online service, the results of the quality and safety of care indicators collected in 2024 and specifies that nearly 70% of certification decisions for health establishments have been made as part of the current cycle. “ These results provide a snapshot at a given moment of the level of quality of care, once treatment is initiated. », Underlines Professor Lionnel Collet, president of the HAS. They thus allow professionals “to take stock of their practices, their difficulties but also their successes. »

Quality and safety indicators of psychiatric care

In 2024, 21 indicators were measured by the HAS, in four sectors of activity: medicine-surgery-obstetrics (MCO), including outpatient surgery, medical and rehabilitation care (SMR), home hospitalization ( HAD) and psychiatry. The data comes, depending on the case, from patient questionnaires (e-Satis), patient files, establishment questionnaires and the medicalization of information systems program (PMSI).

And psychiatry, HAS points out that the life expectancy of a patient suffering from severe psychiatric illness is 20% lower than that observed in the general population. The leading cause of excess mortality is suicide, but accidents and medical causes are also over-represented, particularly cardiovascular and respiratory diseases, the comorbidity rate of which is between 30 and 60%. It is for these reasons that HAS has developed indicators around these themes. On this second measure, we see that the establishments were able to structure their files and that the year 2024 was marked by notable progress linked to the very great efforts put in place on the ground.
– Cardiovascular and metabolic assessment in adult patients is carried out in 69% of patients, i.e. +9 points compared to 2021
– Gastrointestinal assessment is done in 46% of patients1, or +17.5 points
– The identification and offer of help in stopping addictions are effective in 65% of patients (+10.6 points)

Certification: establishments specializing in psychiatry “more in difficulty”

At the end of 2024, nearly 70% of certification decisions for health establishments have been made as part of the current cycle. 87% of establishments show good or very good results. University hospitals and cancer centers obtain the best results and are more likely to obtain the “High quality of care” rating than other establishments. Other types of establishments experience more difficulties. This is the case for establishments specializing in psychiatry.

Perspectives

In this context, additional analyzes are expected in 2025 for hospitalization indicators
full-time in psychiatry. “From next year, the two HAS systems will continue to develop, notably with the establishment of a new standard for the certification of health establishments.
