ARS Île-de- and GIP SESAN announce the opening of the “Santelien” digital coordination file for health professionals in the Ile-de- region

This tool is in addition to the secure instant messaging solution launched in May 2024, to enable better information sharing and easier coordination between all stakeholders. Transversal, the Santélien coordination file is open to professionals in the health, medico-social and social sectors.

Thanks to Santélien, they now have a shared support foldercomparable to a digital connection notebook, in which they can exchange information quickly and securely, organize joint support and follow each of their stages easily.

Santélien is a tool designed in particular for patients whose care and support requires a structured relationship between the city and the hospital or to facilitate their maintenance at home. It also presents numerous features allowing better coordination of the pathways of patients suffering from chronic diseases, or requiring an episode of coordinated care.

Santélien offers a wide range of services for professionals

The GIP Sesan and the Île-de- Regional Health Agency wanted to offer professionals in the health sector and the medico-social sector a new integrated solution, free and reliable, intended for the organization of health pathways and the security of exchanges. Santélien also allows you to quickly identify a resource (professional, structure, care team) and then monitor the progress of the patient’s journey and the follow-up to be given.

This solution includes:

  • Instant messaging (Santelien secure chat)

Instant and secure messaging allows you to join in one click the members of the circle of care around a patient. It meets the needs of professionals to have in their practice a rapid communication tool that complies with the framework applicable to the exchange of health data. The Santélien Secure Chat has been available since May 2024.

The circle of care allows professionals to visualize all the people involved around a patient (professionals, structures and caregivers) and immediately have their contact details available.

  • The digital liaison notebook

The liaison notebook allows the sharing of liaison and coordination notes between professionals in the circle of care: it allows identify and store essential information and useful for monitoring the patient and organizing their care.

  • The task dashboard

This functionality allows you to create tasks for yourself or other professionalswhether they are in relation to a given patient (e.g. call back Mr. Dupont), or organizational (e.g.: schedule the monthly CPTS meeting).

The work list allows you to create thematic lists and configurable patients. These lists can be shared between professionals and thus make it possible to organize a personalized support and organized according to needs.

  • Requesting a resource team

Structures and services that wish to do so can be contacted for a request for care, support or advice, and organize and prioritize the requests received using a monitoring table.

They allow professionals to activate a wide variety of configurable alerts to choose from, depending on their needs and emergencies.

Santélien is gradually connected to the main business tools of health establishments in the region (in particular AP-HP) and city health professionals. This interoperability makes it possible to simplify the management of patient files by avoiding re-entry and by automatically alerting professionals of actions carried out in Santélien or of events in the care pathway (for example hospitalization then discharge from hospital of a patient).

  • A bouquet of services that will continue to expand

Santélien will gradually be enriched with features such as thematic routes or patient access.

  • via the Santélien mobile application for iOS and Android mobile phones and tablets, available on the App Store and Google Play

The account creation methods are adapted to different user profiles and particularly simplified for professionals holding an e-CPS card.

Santélien is the Ile-de-France tool deployed as part of the national e-parcours program.

Complementary to the basic tools of “Ségur du digital” (Shared Medical File, Secure Health Messaging), Santélien is made available free of charge to all professionals in the health, medico-social and social sectors. It is financed by the Île-de-France Regional Health Agency.

In Île-de-France, Santélien is already used by more than 80 health communities (hospitals, clinics, territorial professional health communities, multidisciplinary health homes, coordination support systems, home nursing services, EHPAD , social services, etc.). It already serves several hundred health professionals (doctors, nurses, masseurs-physiotherapists, midwives, pharmacists, caregivers, etc.), and coordinators and social workers for the medico-social aspect.
